Office Policies


The information provided here will assist you in preparing for your appointment. Please take time to read the information and complete the appropriate forms below to ensure a quick visit.

Our staff is very courteous, please be nice to them. They are here to help you.

  • If you have a deductible you will be required to pay in full at each visit or until we can verify that your deductible has been met.
  • You are required to pay your co-pay each visit or your appointment will be rescheduled.
  • Prescriptions cannot be called in after-hours, weekends or holidays, as we need your chart to verify dose and medications. Please plan ahead for your prescriptions.
  • All lab results will be discussed during your office visit not via telephone.
  • Filling out forms – There is a $85 charge for FMLA, Disability, or any other form. We require 5 business days to complete any form. Some employers will reimburse you this charge. 
  • Please call 24 hours in advance to cancel or reschedule your appointment. Otherwise a $75 no show fee will apply.
  • If your call requires that you speak with the physician you will be required to schedule an appointment.
  • Please inform us of changes to your address, phone and insurance.  
  • If you do not provide us with the proper insurance information you will be responsible for the billed amount.
  • Prescriptions, labs slips or authorizations require 2 business days notice to be completed.
  • We ask that you check your prescriptions prior to your appointment so we may assist you with prescription refills. There is a 2 business days turn around for prescription refills. If you have missed an appointment your prescription may be denied.
